Butterfly Visionaries Sponsorship Form

Any organization, group, or individual making a financial or material contribution (including the donation of services) is considered a sponsor. All sponsors will be listed on the website as well as other promotional and/or informational materials. There are levels of sponsorship based on the impact of the donation. To become a sponsor, please fill out and submit the form below. If making a material or service donation, enter the monetary equivalent and description. Material and service donations must go through a review and approval process.

Sponsor (donations of up to $1,000 annually)

Bronze Level sponsor (donations of $1,000 or more annually)

Silver Level sponsor (donations of $2,500 or more annually)

Gold Level sponsor (donations of $5,000 or more annually)

Platinum Level sponsor (donations of $10,000 or more annually)
